Psilocybin can independently cause mild increases in heart rate and blood pressure as well. Combining the two compounds this cardiovascular load and is associated with heightened anxiety, paranoia, and panic in many user reports, on top of the physical strain. This combination is categorized as high-risk in harm-reduction resources rather than an outright "never combine" like lithium or MAOIs, but it still carries real physical danger, particularly for anyone with an undiagnosed or existing cardiovascular condition.
Harm Reduction Guidance
- Avoid recreational stimulants (cocaine, amphetamines not prescribed to you) entirely around a psilocybin session.
- If you take a prescribed stimulant for ADHD, discuss skipping your dose on the day of a session with your prescribing physician, if medically appropriate to do so.
- Consider scheduling a session for a day off from stimulant medication, again in consultation with your prescriber — do not stop a psychiatric medication unilaterally.
- Non-stimulant ADHD medications (such as atomoxetine/Strattera) are generally considered to carry a lower cardiovascular interaction risk, but should still be discussed with a physician before combining with psilocybin.

Seek emergency care immediately for chest pain, severe shortness of breath, an irregular or racing heartbeat that does not settle, fainting, or signs of a panic attack that does not resolve with grounding techniques. These can indicate a cardiovascular event requiring urgent treatment.
